Start the attestation by selecting the appropriate program year. You will be taken to Step 1 – About You. On this screen you will complete three sections.
You need to verify the registration data you entered at CMS. This information provides payee details for your EHR Incentive Payment. If you find errors, you will need to return to CMS and make corrections. Use the link under Featured Program links to return to CMS. Please complete this prior to moving forward with your application.
You will attest that as an Eligible Professional (EP), that you DO NOT perform 90% or more of your Medicaid services in an inpatient or emergency room setting. you will place a check mark – which means that 10% or greater Medicaid services are performed in an Ambulatory setting (local clinic, private practice)
Next, you will verify your Provider Medical License, Medicaid ID and Payee Medicaid ID.
Finally, please update the contact information. We ask for this because it helps us have a person in the office that is aware of how the application was filled out. We also send important program updates, training opportunities and webinar invitations to this person.
